Disability Services Forms
Appointments are available Monday through Friday during normal business hours. The office will respond to your request at our earliest convenience.
Disability Services intake and letter requests are only monitored during regular office hours (Monday through Friday, 8:00 a.m. to 5:00 p.m.). Requests submitted after 5:00 p.m., on weekends, or during Blinn holidays will be responded on the next Blinn normal operation business day.
Forms
- Intake Packet (complete to APPLY for services)
Required to complete when submitting disability documentation and requesting services for first-time students with the Disability Services Office.
- Letter Request Form (complete to RENEW services)
Complete form each semester to renew the letter of accommodation. This form is for returning students who have utilized Disability Services previously.
- Confidentiality of Records
- Consent to Release Confidential Information (for transferring/graduating students)
